All necessary patient forms can be obtained and completed in our office at the time of your visit.

Finances

Because finances should never get between you and the care you want and deserve, we offer a number of financial alternatives. One is sure to fit your needs. We are an ‘insurance friendly’ office and will work to maximize your benefits.

In addition to cash and check, we accept MasterCard, Visa, American Express, and Discover Card for your convenience. Our financial coordinator is happy to discuss additional alternatives with you.  Some of our doctors offer CARE CREDIT, some offer CHASE HEALTH ADVANTAGE, and some do not offer either financing plan.  Please contact our office so we may answer any questions you have about the financial services that are available.
